Common Oral Antibiotics for UTIs

  1. Nitrofurantoin (Macrobid)
    Use: Uncomplicated UTIs.
    Details: A first-line option for simple cystitis. It is not recommended for pyelonephritis or complicated UTIs due to poor tissue penetration beyond the bladder.

  2. Trimethoprim-Sulfamethoxazole (Bactrim)
    Use: Uncomplicated and some complicated UTIs.
    Details: A frequently used first-line choice, but resistance rates in the local population should be considered before prescribing.

  3. Fosfomycin
    Use: Uncomplicated UTIs.
    Details: A convenient single-dose option that is also effective against multidrug-resistant organisms, including ESBL-producing E. coli.

  4. Ciprofloxacin / Levofloxacin
    Use: Complicated UTIs and pyelonephritis.
    Details: These fluoroquinolones are effective for more serious UTIs, such as pyelonephritis, but are no longer first-line due to increasing resistance and potential side effects.

  5. Cefdinir / Cefpodoxime
    Use: Uncomplicated UTIs.
    Details: Oral third-generation cephalosporins may be used, though they are generally less preferred compared to nitrofurantoin or Bactrim for uncomplicated cases.


Conclusion

For uncomplicated UTIs, Nitrofurantoin, Bactrim, and Fosfomycin are the preferred first-line treatments. Avoid using first-generation cephalosporins (such as cephalexin) and Augmentin empirically due to increasing resistance rates.
